Purpose
All health centers must provide enabling services which help ensure access to the required primary health care services as well as facilitate access to comprehensive health and social services.  Enabling services may include but are not limited to: case management, services to assist the health center's patients gain financial support for health and social services, outreach, transportation and interpretive services; and education of patients and the community regarding the availability and appropriae use of health services. Applicants for this funding opportunity are expected to: 1) describe the target population and its need for enabling services; 2) present a service delivery plan that demonstrates responsiveness to the identified needs of the target population; and 3) present a sound business plan that links the goals and objectives from the service delivery plan to the budget.

 Applicant is an existing Health Center Program grantee that receives funding under one or more of the following special populations: Migrant Health Centers (MHC) (section 330(g)), which provide services to migratory and seasonal farmworkers within the service area and their families; Health Care for the Homeless Program (HCH), (section 330(h)), which provide services to homeless individuals and families within the service area; Public Housing Primary Care Grantees (PHPC) (section 330(i)), which provide services to residents of one or more public housing developments within the service area.

 Applicant is an existing Health Center Program grantee that receives funding under one or more of the following special populations:
