- Federal Employees’ Health Benefits (Option 1): To conduct FEHB business transactions, you will need the enrollment code of the health plan of your choice.  You will find this code in the FEHB plan guide.  Carriers may restrict enrollment based on geographic location.  Ensure you meet requirements by consulting the brochure or by calling the telephone number indicated in the carrier's brochure.  Enrollments for self and family coverage or enrollments in which you or eligible family members have group health insurance coverage other than the FEHB plan in which you are enrolling, may only be processed Monday through Friday between the hours of 6:00 am and 6:00 pm CT.  In addition, you will need each family member's SSN and the date of birth.  If you or an eligible family member have other group health insurance, you will need the name of the group health insurance and the policy holder.
