If you have a problem involving a federal agency, my staff and I
are always willing to assist you. Caseworkers in my district office
are on hand to provide you with assistance in dealing with agencies
including: the Social
Security Administration, the IRS,
the Veterans'
Administration, Medicare,
the military,
visa
and passport offices, immigration,
disability, and federal
employment offices. Please understand that while we can assist
you in bringing your case to the attention of the proper officials
and guide you through the process, we cannot direct an agency to
decide a matter in your favor.
In order for me to make inquires on your behalf, the Federal
Privacy Act requires your written authorization. To do this please
fill out the Casework
Authorization Form print it and return it to my office. Please
be sure to include your contact information so my staff will be able
to contact you promptly. Please explain what type of assistance you
are seeking from my office on the Privacy Form in the space
provided, or attach another page if necessary.
NOTE: If you are seeking my assistance with immigration issues,
please fill out and print my Immigration
form. I will also need you to fill out a casework
authorization form. Because of the nature of immigration cases
I need as much information as you can provide. Including both of
these forms will enable my office to process your case as
efficiently as possible.
