Everyone should be treated fairly. If you live in Utah and have a problem that involves a federal agency, my staff and I will do everything we can to try and assist you.  If you need help determining the status of your benefits or eligibilty for a federal program, I can also help you.

My staff is trained and dedicated to respond to your problems with the federal government on a case-by-case basis. They work on my behalf to identify the problem and act as a liaison with the federal agencies.

If you would like my office to contact an agency on your behalf, please fill out the privacy release form and return it via mail or fax to the field office closest to you or by fax to my Washington office at (202) 224-6331. Make sure to include a brief and clear description of your problem and copies (DO NOT SEND ORIGINALS) of any relevant documents.
