CIRCULAR LETTER NO. 710

TO: Printing and Publishing Officials of the Federal Government

SUBJECT: Rider Requisitions for the HHS Publications “Physical Activity Guidelines Materials”

The Government Printing Office (GPO) is now accepting rider orders for the U. S. Department of Health and Human Services publications “2008 Physical Activity Guidelines for Americans,” “Be Active Your Way—Consumer Booklet for Adults,” and “Consumer Booklet Fact Sheet.” These publications help Americans use physical activity to improve their health and fit physical activity into their lives.

The estimated rider rate for these publications is $3, $1 and $2 per copy respectively, which applies to single destination, local delivery. Please note that the “Consumer Booklet Fact Sheet” is being produced in pads of 50 sheets and the estimated cost is per pad. Mailing charges, if incurred, will be added to your account. Requisitions should be forwarded to Agency Publishing Services, Requisition Section, Mail Stop CSA, U.S. Government Printing Office, Washington, DC 20401, by November 18, 2008. Please be sure to indicate GPO Jacket No. 350-590 on your requisition for “2008 Physical Activity Guidelines for Americans,” Jacket No. 350-591 for “Be Active Your Way—Consumer Booklet for Adults,” and Jacket No. 350-592 for “Consumer Booklet Fact Sheet.” A separate requisition is required for each publication you wish to ride. Field or regional activities should coordinate their requirements with their Washington, DC, area headquarters’ printing officers before submitting any paperwork.

Requisitions should be submitted by one of the three following methods: Addressed and mailed to Agency Publishing Services, Requisition Section, Mail Stop CSA, U.S. Government Printing Office, Washington, DC 20401; emailed to requisition@gpo.gov; or efaxed to (202) 478-1762. Your agency’s authorized personnel as reflected in our agency signature file must sign all requisitions submitted.
