If You Need Immediate Assistance
You need to write a letter containing the pertinent details of your problem and send it to my office. Please include your name, Social Security number, address, city, zip, signature and daytime phone number. You’ll also need to print out and sign our Authorization Form giving us the authority to begin working on this problem.
