                  Disciplinary Action Report

 

 

 

 

The following Disciplinary Action Report (DAR ) link contains a list of final disciplinary actions taken by the Oregon Department of Human Services, Public Health Division, Emergency Medical Services and Trauma Systems (EMS/TS) Section regarding First Responders, EMTs and EMS Agencies during the time period specified. The DAR also includes a summary of the disciplinary action taken.  The complete document may be obtained by making a public records request. The DAR does not include Letters of Instruction, Letters of Reprimand, or Civil Penalties.  Beginning February 1, 2007, the DAR will be updated monthly and will include final actions taken by the EMS/TS Section during the previous month. Information concerning final disciplinary actions taken prior to January 2007 can be obtained by making a public records request.

 

If you would like to make a public records request regarding final disciplinary actions, please put your request in writing and send to:

Alternatively, you may send your request via e-mail to Liz Morgan, Prehospital Standards Unit - elizabeth.e.morgan@state.or.us. Your request should include the name of the First Responder, EMT or Agency in question, a telephone number or e-mail address where you can be contacted, and a return address. Requests for records will be subject to a copying fee of 25 cents per page.

 

The EMS/TS Section has developed a handbook entitled "The Prehospital Standards Process," which helps explain the disciplinary process utilized by the Section. It is a good source of information regarding the disciplinary process and can be accessed at: http://oregon.gov/DHS/ph/ems/profeson/handbook.pdf.

 

The DAR is not intended to fully describe the disciplinary actions taken by the EMS/TS Section and information contained in the report should not be used for official purposes.
