DMV offers Record Inquiry Account Holders the option of submitting a request for driver or vehicle records via the internet.

Oregon DMV provides forms and other publications in Portable Document Format (PDF). Version 3.0 or better of Adobe Acrobat Reader is required to view and/or print them.
Fillable PDF Forms
DMV offers selected PDF forms as "fillable", where you can enter data into the forms on your computer screen, print them on your own printer, sign them, and mail or take them to DMV.
