Submitting Forms
Only forms that are designated as "Online" (instead of a form number) trigger real time data entry and submittal.
 
Only forms that are designated as "Fillable" (instead of a form number) trigger real time data entry and downloading or printing.
 
Fillable and standard forms can not be submitted electronically.  They are provided so that you may fill them out and mail them to DMV or take them to a DMV Office.
 

Portable Document Format (PDF)
Oregon DMV provides forms and other publications in Portable Document Format (PDF).  Version 3.0 or better of Adobe Acrobat Reader is required to view and/or print them.

Fillable PDF Forms
DMV offers selected PDF forms as "fillable", where you can enter data into the forms on your computer screen, print them on your own printer, sign them, and mail or take them to DMV.

Note: Some printers may require use of the "Shrink to Fit" printer dialog box to be checked in order to print forms on a single page.
