FAQs about becoming a DMAP outreach site

 

Q.  Who can become a contracted outreach site?

A.   DMAP only contracts with providers of direct health care services. Contracted sites include:

  • County health departments
  • Hospitals
  • Rural health clinics
  • Tribal health clinics
  • Federally qualified health clinics
  • Family planning clinics
  • A&D detox centers for adults
  • Youth residential detox centers

Q.  What do outreach sites do?

A.   Contracted outreach sites:

  • Distribute OHP applications to people using their clinics
  • Assist people in completing applications
  • Verify citizenship documents
  • Date stamp applications to establish the date of request

Q.  Why should our office become an outreach site?

A.   To ensure your uninsured patients apply for Oregon Health Plan coverage and establish a date of request on the day you render health care services.

 

Q.  How can we become an outreach site?

A.   Submit a completed application (DMAP 6600). If your facility meets the criteria set by DMAP, we will send you a contract to sign. You will need to identify your authorized signators. Some entities must have approval from county commissioners, and some entities require their own legal review.

 

Q.  What assistance will we receive to begin operation as an outreach site?

A.   Once we have a fully executed contract, DMAP will send your facility:

  • A welcome letter with an assigned facility code
  • A training schedule
  • OHP materials order forms
  • Quarterly reporting forms
  • Information on ordering a date stamp

Q.  How do I get OHP applications and brochures?
A.   Complete and fax form DMAP 6625 to the number on the form.
