UNOS provides an OPTN member reporting line (866-787-4909) for transplant professionals to report potential patient safety issues or OPTN policy violations at member institutions.

UNOS staff monitors the reporting line from 8:30 a.m. to 5:00 p.m. EST, Monday through Friday, and maintains confidentiality to the extent permitted by law. Any calls received after business hours are forwarded to a voice messaging system.
