Driver
License Application (DLD-1)
For an original Nevada driver license. Must be submitted in person
at a DMV Office.
Do Not Mail This Form! Mailed applications are rejected.
Minor
Affidavit (DLD-38)
Certifies that a minor is aware of various requirements and laws.
Also used by a parent/guardian to cancel a minor driver license or instruction
permit.
For Commercial Driver License US DOT exam requirements.
Current license holders who are filing
only an updated physical may mail the form to the
nearest CDL
Office or deliver it in-person at any CDL or Full
Service Office.
Please note that commercial drivers must also have a Medical Examiners Certificate completed by their physician. These are not
available online. You may obtain one from a DMV CDL

This form is not necessary for family sales completed on or after January 1, 2006.
Motor Vehicle Gift Affidavit (MVH-GFT-01.01) Used to request a Sales Tax Exemption on vehicles given as bona fide gifts between non-family members. Must be submitted to the Nevada Department of Taxation. This form is not necessary for gift transactions completed on or after January 1, 2006.
Used to adjust registration fees on large farm vehicles.
Vehicle
Inspection Certificate (RD-15)
For Vehicle Identification Number Verification. Must be completed by a DMV representative, a peace officer or other
authorized agent.
Dormant Vehicle
Affidavit (IVP-003)
Used to reinstate a vehicle registration which has been suspended for failure to maintain
liability insurance. Only used if the vehicle has not been operated during the lapse of insurance.
This form is for use only if the vehicle was last
titled in Nevada. It must be completed in full, including the date the
original title was issued.
If you are not sure whether your vehicle is titled in Nevada or if you do
not have all of the information requested, please contact the Records
Section at (775) 684-4590.
If your vehicle is titled in another state, you must obtain a duplicate
from that state.
You must file this form within 10 days of any accident which was: (1) not investigated at
the scene by law enforcement and (2) caused injury, death or more than $750 damage.
Used to file complaints against auto dealers, lessors, emissions stations, driving schools
