Change of Address Application (DLD-22)

Fill-In Form  This Form Is Available By Fax

Note: Address changes on Commercial Driver Licenses must be completed in person.
Motorists are required to notify DMV of a change of address within 30 days of the move. You can mail in the form or fax it to (775) 684-4992. You will receive a sticker for the back of your driver license by return mail. There is no charge. See also Address Changes. Military members stationed outside Nevada should include an LES.

Updated Registration Certificates cost $5 each. Include a Credit Card Authorization with faxed applications or an authorization, a check or a money order with mailed applications.

Driver License Application (DLD-1)
For an original Nevada driver license.  Must be submitted in person at a DMV Office. 
Do Not Mail This Form!  Mailed applications are rejected.

Application for Nevada Driver License by Mail (DLD-4)

This Form Is Available By Fax

For drivers currently licensed in Nevada who are temporarily out of state.
Cannot be used to obtain an original license.

To claim exemption from federal commercial licensing regulations.

Physical Evaluation Form (DLD-100)
For renewal applications when a physician or optometrist examination is required.

For visual examinations completed by a physician.

Application for Expedited Service Permit (SP-59)
For disabled individuals.

Confidential Organ Donor Registration
Consent to be listed in the Nevada Organ Donor Registry and authorization for anatomical gifts.

Application for Restricted License - Employed/Unemployed (DLD-10 & DLD-21)
To apply for a limited license during a license suspension or revocation for DUI or points suspension.

Child Support Restricted License Application (DLD-25)
To apply for a limited license during a license suspension or revocation for failure to pay child support.

Declaration of Veteran Status (DLD-129)
Used only to help the Nevada Office of Veterans Services track the number of veterans in the state.

Declaration of Homeless Status (DLD-128)
For the homeless to apply for a one-time waiver to obtain a duplicate license or ID card.

Minor Affidavit (DLD-38)
Certifies that a minor is aware of various requirements and laws.
Also used by a parent/guardian to cancel a minor driver license or instruction permit.

"To and From" School Application (DLD-9)
To enable minors to drive to school when there is no other practical means of transportation.

Minor Application to Transport Medically Disabled (DLD-3)
For minors to transport disabled individuals to medical appointments.

For use by physicians who are evaluating a license applicant's medical fitness to operate a motor vehicle.

One Year Default Affidavit (DLD-33)

For drivers whose license is suspended under financial responsibility laws.

There is a $5 fee for each certificate requested. You may mail this application and a check, money order or a Credit Card Authorization to the address on the form. Or, you may fax the form and credit card authorization to (775) 684-4797.

Emission Control Residency Affidavit (EC-8)

  Fill-In Form  This Form Is Available By Fax

For Nevada residents temporarily out of state.  
Satisfies smog check requirement for registration renewals.

Family Sale/Gift Affidavit (RD-173)

  Fill-In Form  This Form Is Available By Fax

Used to request a sales tax exemption on vehicles transferred between family members.
This form is not necessary for family sales completed on or after January 1, 2006.

Motor Vehicle Gift Affidavit (MVH-GFT-01.01)
Used to request a Sales Tax Exemption on vehicles given as bona fide gifts between non-family members. Must be submitted to the Nevada Department of Taxation. This form is not necessary for gift transactions completed on or after January 1, 2006.

Affidavit of Non-Operation (RD-18)

Certifies that a specific vehicle has not been operated on Nevada public highways.

Designation of Use for Veterans Exemption (RD-187)

Application for Individual Record Information (IR-002)
Anyone who requests driver license or registration records must complete this packet.  Motorists who need copies of their own records should visit a DMV office or contact the Records Section as listed in Driver History.

Application for Records Service (IR-001)
For insurance companies and other businesses which request individual DMV records on a regular basis.

Please contact the Records Section for fee quotes and other details if you desire statistical reports or a large number of individual records. The release of DMV records is governed by Nevada Revised Statute 481.063 and the National Driver Privacy Protection Act.
