Based on your feedback, there could be problems with a particular batch of FD-249, Arrest and Institution Fingerprint (Criminal) Cards.  Some agencies have reported they are having a hard time getting the cards to run through their printers.  If you are experiencing similar problems,  please check the order number on the back of the card.  If the last five numbers at the bottom left hand corner is 80001, destroy these cards and reorder more.  You may fax your order to 304-625-3984 or order online at   If you need a copy of the new order form or if you have any other questions please contact us at 304-625-3983.  We apologize for any inconvenience this may have caused you.
