DC, FBCO Contract Sample
The D.C. Department of Employment Services
entered into legal contracts with each of its FBCO sub-grantees to implement a
job readiness and job placement program to serve hard-to-serve adults providing
essential supportive services, including monitoring, personal support,
clothing, child care, transportation and other ancillary services. This sample
shows how to structure a contract with an FBCO including a description of
services and tasks, staffing responsibilities, reporting, records, monitoring,
budget, payment provisions, financial management and audit, records and
reports, and other terms and conditions.

Arundel, MD, FBCO Contract Sample
The Anne Arundel Workforce
Development Corporation entered into legal contracts with each of its FBCO
sub-grantees. For this project, all clients had to be low-income adults
residing in public housing or receiving housing subsidies, or receiving other
public assistance. The contract stipulates that the grantee needs to host an
on-site publicly accessible Mini-Career Center; provide core and intensive
services to no less than 100 eligible customers; enroll and fund no less than
40 eligible customers in training services; and several other terms are listed.
This sample shows how to structure a contract with an FBCO including the
statement of work, legal conditions, budget, and payment terms. It includes a
description of services, program operation hours, client eligibility,
enrollment, performance measures, deliverables, and other program requirements.

Group Operations Manual
The SkillSource Group, Inc. is the non-profit
of the Northern Virginia Workforce Investment Board and developed this manual
for its 15 sub-grantees based on their experience that a lot of questions come
up with a new grant. The manual explains the NVWIB's mission along with a map
of SkillSource Centers; provides information on the Department of Labor's
Center for Faith-Based and Community Initiative, equal treatment regulations
for religious organizations, and definitions for outcomes measures; and
includes case management documents and financial reporting instructions. Other
templates include a referral form, individual employment plan, eligibility
verification, needs assessment, client satisfaction survey, monitoring tool,
and additional forms. Several of the sub-grantees are working with Limited
English Populations and the forms in the manual will be translated.

Operations Manual
Anne Arundel Workforce Development Corporation
created a practical guide for its sub-grantees to ensure program consistency.
This manual was distributed to each sub-grantee at an orientation, where they
reviewed the entire manual together. The manual includes all relevant
information about the grant program as well as templates needed by
sub-grantees. The manual explains AAWDC's mission and organization; provides
information on the Department of Labor's Center for Faith-Based and Community
Initiative, equal treatment regulations for religious organizations, and
definitions for outcomes measures; and offers guidance on program management
and financial reporting. Templates include a participant sign-up sheet, case
management checklist and narrative, individual employment plan, training
eligibility form, and others. The grants administrators also included a
monitoring tool designed to monitor the sub-grantees and explained the
monitoring process.
