Description:
Objective:The objective of the University of Southern California (USC) Children’s Environmental Health Center is to develop a better understanding of how host susceptibility and environmental exposures contribute to children’s respiratory disease. This research will provide health and environmental officials with a variety of useful intervention tools. USC intends to investigate the relationship between environmental tobacco smoke (ETS) and asthma in children. This Center will determine the biological mechanisms by which ETS alters normal allergic responses in children.
The Administrative Core of the Children’s Environmental Health Center (CEHC), located at the USC Keck School of Medicine, supervises and coordinates a multidisciplinary and multi-institutional effort for the research and outreach in children’s environmental health issues, in particular, as they relate to air pollution and respiratory disease and prevention.
The Community Outreach and Education Program (COEP) of the CEHC attempts to connect an interdisciplinary team of scientists from USC and the University of California at Los Angeles (UCLA) with other researchers studying issues of children’s environmental health, as well as with community-based organizations and others interested in children’s environmental health issues. The COEP program at the Keck School of Medicine of USC began when the National Institute of Environmental Health Sciences (NIEHS) funded the Southern California Environmental Health Sciences Center (SCEHSC) in 1996. The CEHC has allowed an expansion of partnerships with community-based and public health organizations, as well as with health care providers and schools, particularly around issues of asthma.
The primary objectives of CEHC’s COEP are to: (1) create awareness of new problems and research opportunities in children’s environmental health that are relevant to the community; (2) collaborate with community-based organizations and community-focused institutions on projects involving children’s environmental health issues; (3) provide support to Center members by linking their research efforts with interested community organizations; (4) identify and respond to programmatic needs and requests concerning environmental health issues that affect children that are raised within the community; and (5) promote education about children and environmental health issues in kindergarten to grade 12 (K-12) school programs.
Progress Summary:Administrative Core
The CEHC developed new and ongoing collaborations and accomplishments both within its structure and with USC, UCLA, and the community as part of its research and outreach missions. The research and community outreach accomplishments are described in a subsequent section. Specifically, the Administrative Core was involved in the following activities:
- Dr. Gong and several others (Drs. David Diaz-Sanchez and Rob McConnell) attended the second annual meeting of the Directors of the CEHC during October 1999.
- The External Advisory Committee remains unchanged. Dr. David Schwartz remains on the Committee despite moving to a new position at Duke University. The Committee last met on October 14, 1999, and provided the Center faculty with helpful suggestions. The Committee is scheduled to meet again on November 20, 2000.
- The principal investigators (PIs) of the three research projects and the Internal Advisory Committee met quarterly to review their research progress.
- The New Scientist Program remains under development and review because of financial restrictions.
- The CEHC Web Site currently is under development by Andrea Hricko and Carrie Van Doren.
- Interactions between the CECH and the NIEHS-supported SCEHSC continue to occur. Several CEHC investigators presented their research in the SCEHSC’s established research seminar series, and additional CEHC presentations are planned. The CEHC investigators made use of the SCEHSC’s Facility Cores, such as storing buccal samples (by Dr. Frank Gilliland) in the Biological Sample/Processing Core, accessing advice about ETS generation and monitoring (by Dr. Diaz-Sanchez) from the Analytical Chemistry Core, and obtaining statistical advice (by all PIs) from the Biostatistics Core.
Significance
The CEHC has allowed expansion and synergism of research and community interactions that, heretofore, were not present in our institutions and communities.
Community Outreach and Education Program
The COEP has developed a number of significant ongoing collaborations that facilitate CEHC’s research and outreach on issues involving children and environmental health issues.
CEHC collaborated with the following partners during the Keep Improving Control of Kids’ Asthma in Los Angeles (KICK Asthma LA) program:
- Mothers of East Los Angeles—Santa Isabel.
- American Lung Association of Los Angeles County.
- City of Los Angeles Environmental Affairs Department.
- Environmental Protection Agency—Child Health Champion Program.
- Francisco Bravo Medical Magnet High School.
- Murchison Elementary School.
- School nurses at nine elementary schools in El Sereno.
- Various health clinics and hospitals.
- Local businesses.
The CEHC also collaborates with the following organizations:
Environmental Health Coalition (EHC), San Diego. The CEHC faculty and staff are advising the EHC on issues dealing with children and air pollution. Ed Avol has been working with the EHC on placement of an air monitoring station in Barrio Logan, a poor community of San Diego.
Boyle Heights Mejoramiento. Ms. Hricko has provided information on traffic and noise pollution to a new Boyle Heights Group, formed in January 2000, which is concerned about the effect of truck exhaust and noise pollution on school children’s respiratory health in an area of dense diesel truck traffic.
Communities for a Better Environment (CBE). The CEHC outreach director is working with CBE in developing a section of its environmental leadership training manual to deal with asthma issues, as part of the collaboration of the asthma intervention study directed by Drs. Craig Jones and McConnell. In addition, under a joint NIEHS Environmental Justice: Partnerships for Communication grant, the COEP and CBE have conducted surveys in Huntington Park health clinics to assess whether asthma is perceived as a serious problem in children and whether clinic staff would be interested in attending luncheon seminars on asthma management. The first seminar is scheduled for September 19, 2000, and asthma specialists from the local community will be speaking.
The following are highlights from Year 2 of the CEHC Grant:
Participation in a Community Team To Address Children’s Environmental Health Issues in East Los Angeles. The COEP is a key partner in KICK Asthma LA, a collaborative effort designed to empower a community on the east side of Los Angeles to protect its children from environmental health threats, especially asthma. The COEP has played a leadership role (with Mothers of East Los Angeles, the American Lung Association of Los Angeles County, and the City of Los Angeles’ Environmental Affairs Department) in forming a community team that consists of representatives from community-based organizations, the Los Angeles Unified School District, local government agencies, public health organizations, and businesses, along with elected officials, community residents, students, student nurses, and others.
Children living in the East Los Angeles community adjacent to the USC Health Sciences Campus are exposed to a wide range of environmental health threats. The team considered environmental health threats in the community and selected asthma as its priority. The team has been providing Open Airways Training in eight elementary schools in the area and has been recruiting families of children with asthma to participate in home visits in which parents receive instruction on how to reduce their child’s exposure to asthma triggers. This effort, funded in part by the U.S. Environmental Protection Agency, involves COEP staff and CEHC scientists in a close working relationship with a number of community organizations, as well as city government, high school students, student nurses, school principals and nurses, and local businesses.
K-12 Activities. During the summer of 2000, the COEP director brought in a high school student from the Health Academy, under a special Keck School of Medicine of USC fellowship program, to learn skills of conducting community-based participatory research and to assist in helping COEP staff better understand the perspective of local residents about children’s environmental health problems in their neighborhood.
Assisting Boyle Heights Mejoramiento (Making Boyle Heights Better). The COEP is helping this group in its efforts to reduce noise and diesel exhaust levels in the Boyle Heights neighborhood, not far from the USC campus. The community is impacted by a confluence of freeways and by surface streets that carry dense truck traffic. The residents particularly are concerned about noise levels affecting school children’s ability to learn, because there are five schools near the freeways and no sound walls to buffer the noise from the classroom. Residents also are concerned about diesel particulate exposure. The COEP Director is working with this group to monitor noise levels and to interpret air pollution sampling results.
Future Activities:Future plans of the Administrative Core are to: (1) further the progress of research projects; (2) continue to develop the New Scientist Program; (3) continue to develop the CEHC Web Site; and (4) participate with other Children’s Centers in the Annual Meeting of the American Public Health Association in Boston, Massachusetts, during November 2000.
The COEP anticipates continuing its asthma-related work in the community and offering its assistance to local groups concerned about environmental health problems. It will broaden its environmental justice activities through the recent funding of the NIEHS Environmental Justice: Partnerships for Communication grant to EHC, in which Ms. Hricko, Dr. McConnell, and Mr. Avol are partners.
