APTI Registration Procedures and Forms

All self-instructional courses are available on-line in PDF format If you do not need to receive CEU credits for a course, you can study the course material, take a test and upon passing the test, you can print a certificate at your desktop. For this, no registration with APTI is needed.

If you would like to receive CEU credit for a course, use the table below to determine if you are eligible and to understand how CEUs are credited.

For Classroom Courses

• Registration procedures vary among sponsoring organizations and course providers. It is the responsibility of each student to contact the sponsoring organization or course provider to confirm registration procedure. • Consult the APTI Training Calendar for specific course dates and times.

For Satellite Broadcasts

• Anyone who wishs to attend a broadcast must contact the site coordinator where you plan to attend. To determine the nearest downlink site to you, refer to the site locator map or contact Carmelita Harrisonat (919)541-5516.
• Only 300 and 400 level telecourses require registration with APTI (example course T401-xx).
• Registering for a satellite based 300-400 level course is a two-step enrollment process.
  1. Register to become a student with APTI. To register as a student, you must complete the Application for Training form. If you have already completed the Application for Training form and only need to make a change in your personal information (name, address/phone number, or PIN), please use the Information Change Request form.
  2. Register for the course of your choice. To register for a course, you must complete the Class Registration form.
• All other satellite broadcasts do not require registration with APTI.
• A certificate of completion for 300- and 400-level telecourses will be issued to all registered students who score 70 or above on the telecourse final exam. Exams must be received within two weeks from the last day of the broadcast.

APTI Forms

Application for Training Use this form to register with APTI and to obtain a PIN. The PIN is a self assigned number (max 10 digits). If the PIN you choose is already used you will be notified. This form should only be completed one time.

Information Change Request Use this form to update information such as name, address, phone etc with APTI.

Class Registration Use this form to register for level 300-400 satellite broadcast courses. For more information on how to register for level 300-400 satellite broadcast courses, see ‘For Satellite Broadcast’ above.
This form does not need to be completed for self instructional courses that are taken online.

Request for Transcript Print, complete, sign, and fax or mail to the address listed below. A copy of the APTI courses completed will be sent to you.
