Civil Service New Employee Entrance On Duty (EOD) On-Line Package


Below you will find a list of standard forms that must be completed in order for us to bring you on board to work in the Agency. Please complete the forms that pertain to you and bring them with you on your first day of work. (Note: Internet addresses are provided so that you can access valuable information on the different benefits available to you as a federal government employee.)

Federal Employees Health Benefits (FEHB):


Flexible Spending Accounts (Pre-tax Benefit Program)
New and newly eligible employees have 60 days from their entry on duty to enroll in this program. Enroll on line


The Federal Long Term Care Insurance Program
Check the Website to Verify your eligibility to enroll -- http://www.opm.gov/insure/ltc/index.asp. Enroll on line. You (and your spouse, if you're married) have 60 days from your entrance date to apply for Long Term Care Insurance using the abbreviated underwriting application with only a few health-related questions. If you apply AFTER the 60 day period, you will have to use the long underwriting application with numerous health-related questions, and possibly a review of medical records and/or an interview with a nurse.


Federal Employees Group Life Insurance (FEGLI):

  • SF-2817 (pdf) - FEGLI Election (return within 31 days of hire date) - Only New Federal Government Employees

Enrollment forms and miscellaneous information regarding Federal Government benefits. (Employees transferring from other federal government agencies will continue to be covered under their existing benefits coverage and need not to re-enroll.)

SF-2808 (pdf) - CSRS Designation of Beneficiary (Not for employees new to the federal government) -- All CSRS employees Note: TSP-50, Investment Allocation Form, is not available in electronic format because the form is designed to be read by an optical scanner. You can obtain a copy of the form from the Human Resources Office on your entry date.
