The GSA, Office of Inspector General (OIG), maintains the GSA IG Hotline to facilitate reporting of allegations of fraud, waste, and mismanagement in GSA programs and operations of the agency.

If you wish to remain anonymous, omit all questions marked "Optional."  However, please be advised that your e-mail address and message that are submitted to GSA/OIG  via this form may not be confidential because of the inherent openness of the Internet.

Describe your complaint by answering the following questions:

Who did it? Include names, addresses, and phone numbers of individuals or GSA contractors involved.

When did it happen?

Which GSA program or office was involved?

What did they do that was wrong and how did they do it?  Please describe the behavior or event in as much detail as possible. (Mandatory field)

Do you have first hand knowledge of the event?

Where can we obtain evidence of this event?

Who else knows about the event and how do they know?
