Description:
Objective:The broad objectives of the Center’s research activities during its first 5 years were:
(1) To identify childhood determinants of respiratory susceptibility to environmental factors with a focus on ambient air pollution and secondhand smoke;
(2) To identify the modulating effects of secondhand smoke on allergic inflammation in children; and
(3) To determine the feasibility and efficacy of a community-based intervention to reduce adverse health outcomes from co-exposures to indoor allergens and secondhand smoke in a high-risk group of children.
Summary/Accomplishments (Outputs/Outcomes):The Children’s Environmental Health Center (CEHC) was established in 1998 as a specialized center for investigating the effects of the environment on children’s respiratory health with a focus on asthma and allergic airway disease. The theme of the Center and its research focus were chosen based on the clinical and public health importance of childhood respiratory diseases and the potential importance of common environmental exposures on the etiology of these conditions, especially among susceptible subgroups of children. In 1998, the Center also established a community outreach and education program to translate research findings into education and public health policy. During the first 5 years of the CEHC, we made substantial progress in understanding the effects of ambient air pollutants and tobacco smoke on children’s respiratory health and identified important susceptible groups of children. The specific accomplishments are summarized in the Final Reports for R826708C001–R826708C003.
Conclusions:The key findings for the first 5 years of Center research are:
- In utero exposure to maternal smoking increased the risk of asthma, especially among children with two phase II enzymes variant genotypes: GSTM1 null and GSTP1 A105G (Gilliland, et al., 2003a; Gilliland, et al., 2002a).
- High levels of exposure to locally emitted fresh vehicle exhaust were associated with increased risk of early life asthma (Gilliland, et al., 2002a).
- The incidence of newly diagnosed asthma is increased in children playing team sports in high, but not low, O3 communities (McConnell, et al., 2002).
- The TNF–308 genotype is associated with early life asthma prevalence, but the effect of the genetic variant is limited to low O3 communities (Gilliland, et al., 2003b).
- Early life experiences including exposures to farming, pesticides and herbicides, cockroaches, and early day care were associated with increased risk of asthma onset before age 5 years (Gilliland, et al., 2003c).
- In utero exposure was associated with deficits in lung function that were larger in children who were diagnosed with asthma early in life (Gilliland, et al., 2000).
- Chronic lung function deficits occur in children with asthma, especially those with early diagnosis of asthma (Berhane, et al., 2000).
- Overweight and obese children are at increased risk for new onset asthma (Gilliland, et al., in press a; Gilliland, et al., 2001a).
- Increased prevalence of bronchitic symptoms occur among children with asthma with higher exposures to ambient NO2 and particulate matter, especially the elemental and organic carbon fractions (McConnell, et al., 1999; McConnell, et al., submitted a).
- Deficits in lung function growth with nitrogen dioxide (NO2) levels (and a cluster of highly correlated pollutants, including nitric acid vapor and particulates) (Gauderman, et al., 2000). This result has also been replicated in our second cohort of fourth graders enrolled 3 years later (Gauderman, et al., 2002).
- Improvement in the rate of lung function growth in subjects who moved to areas of lower PM10 compared with those who moved to areas of higher PM10 (Avol, et al., 2001a).
- Diets low in antioxidant vitamins or magnesium are associated with chronic deficits in lung function (Gilliland, et al., 2001b).
- There is a large increase in respiratory-related school absences with daily fluctuations in O3 levels, particularly in low PM10/NO2 communities (Gilliland, et al., 2001c; Berhane and Thomas, 2002).
- There is increased risk of respiratory-related school absences among children with and without asthma exposed to secondhand smoke (Gilliland, et al., in press b).
- Risk of respiratory-related school absences varied by GSTM1 and GSTP1 genotype (Gilliland, et al., 2002b).
- Secondhand smoke exposure has adjuvant effects on allergen induced nasal allergic responses that is 10-fold larger in children than adults (Diaz-Sanchez, et al., 2002).
- Risk of asthma is increased among children with a dog, but not a cat, in the home.
- The adjuvant effects of diesel exhaust particles (DEP) on nasal allergic responses strongly depends on GSTM1 and GSTP1 genotype (McConnell, et al., 2003a).
- Nasal allergic responses to DEP are reproducible and identify a subpopulation of children with enhanced allergic responses from DEP (Bastain, et al., 2003; Bastain, et al., accepted).
- Methods and materials for community-based interventions to reduce indoor allergens in the homes of children with asthma can be developed and implemented with community involvement (McConnell, et al., 2005; McConnell, et al., 2003b).
- Cockroach counts can be reduced by a community-based intervention using Integrated Pest Management (IPM) (McConnell, et al., 2005; McConnell, et al., 2003b).
- Parents of children with asthma who have been tested for allergies using skin prick test are well informed by clinicians about the test results (Richardson, et al., 2004).
- Indoor allergen and endotoxin can be measured using dust wipes that could be self-collected by participants in research studies (Avol, et al., 2001b; Thorne, et al., 2005).
- Communities are concerned about the effects of ambient air pollution and local traffic on asthma and want to be involved in understanding and reducing any risks to their children.
Taken together, these findings support the conclusions that in utero and childhood exposures to tobacco smoke and childhood exposure to ambient air pollution at current levels have adverse effects on children’s airways.
