Notice: FDA Form 3356 Annual Tissue Establishment Registration

Please be advised that FDA will not be mailing the annual registration form to establishments that register electronically. A reminder announcement will be sent to the email address of the reporting official.
- All establishments are required to submit annual updates to FDA by December 31, 2006 regardless of changes.
- It is important that the reporting official's email address is correct and not blocked from mail received from the tissuereg@fda.hhs.gov account
- FDA is encouraging all establishments to address their annual registration electronically.
- When registering electronically, remember to always hit the SUBMIT TO FDA button in order to submit your annual registration to FDA.
