Q : How are the vaccination results reported to USCIS?

Civil surgeons and, in the case of refugee adjustment applicants only, state or local health clinics must report the vaccination results on the supplement to Form I-693 (also known as vaccination supplement). 

Refugees and K and V nonimmigrant visa applicants need to only submit the vaccination supplement with the adjustment application since the Form I-693, Medical Examination of Aliens Seeking Adjustment of Status should have already been submitted when they applied for the visa or admission.
