Description:
Objective:The University of California–Berkeley Center for Children’s Environmental Health Research is a community-university partnership investigating the environment and health of primarily low-income, Latino children living in the Salinas Valley, an agricultural community in Monterey County, California. Partners in this project include the local and state health departments and environmental agencies, community clinics and advocates, farm workers, and representatives of the agricultural industry. Under this grant, we will assess exposures and health outcomes in the children through age 7 who are participating in the Center for Health Assessment of Mothers and Children of Salinas (CHAMACOS) study. The CHAMACOS study is a longitudinal birth cohort investigating pesticide and allergen exposures to pregnant women and their children living in an agricultural community. We also will conduct specialized pesticide exposure studies to improve our understanding of pesticide metabolism and excretion during the pregnancy and birth, identify the best samples to characterize exposures for epidemiologic studies, and evaluate the contribution of diet to total pesticide exposure in young children. We also will conduct innovative in-vitro laboratory studies investigating cell responses to mixed exposures of pesticides and allergens. Finally, we will develop a Community Outreach and Translation Core (COTC) to ensure that study findings are widely disseminated to local, regional, and national audiences so that appropriate policy responses to improve children’s health are supported.
Progress Summary:In the past year, we have completed the 42-month contact with our participants and have begun planning our contacts with families when the children will be 60 months old. We also have conducted a home-based education study to reduce pesticide exposure in farm worker families and a study to elevate the comparability of spot and 24-hour urine samples for estimating pesticide exposure. We are continuing data analysis and have published several papers. Scientific highlights in the last year include:
- We completed a pesticide exposure validation study, including multiple spot and 24-hour urine samples from 25 children. We found that same day spot and 24-hour urine samples were correlated (spearman r = 0.6-0.7) but that 24-hour samples collected 3 days apart were not correlated. These data address critical questions about the ability of urine samples to characterize children’s pesticide exposure. Preliminary results were presented at the International Society of Exposure Analysis (ISEA) meeting in October 2004 and are relevant directly to many of the Children’s Centers and the National Children’s Study.
- We completed a study of the association between organophosphate pesticide (OP) exposure during pregnancy and shorter length of gestation and the association of in utero organophosphate pesticide exposure and fetal growth and length of gestation in an agricultural population. We found no adverse effects on fetal growth. Gestational duration, however, was decreased in mothers with higher levels of two different measures of OP exposure during pregnancy. Women with higher levels of urinary dimethyl metabolites or lower levels of acetyl cholinesterase in umbilical cord blood had significantly shorter gestational duration (β = -0.41 weeks per log10 unit increase, p = 0.02 and β = 0.34 weeks per unit increase, p = 0.001, respectively).
- We studied the association of OP exposure during pregnancy and abnormal reflexes in neonates. We completed Brazelton Neonatal Behavioral Assessment Scales (BNBAS) on 381 newborns. We found that increasing maternal urinary metabolite levels were associated with both an increase in number of abnormal reflexes [β = 0.53, p < 0.05] and the proportion of infants with more than three abnormal reflexes [OR = 4.9, p < 0.05]
- We completed a paper characterizing housing quality in our population. Severe housing disrepair and crowding was highly prevalent in our population compared to Department of Housing and Urban Development (HUD) national reference data. Further, housing disrepair and crowding was significantly associated with cockroach or rodent infestation. Interventions addressing pest infestation must include improvements in housing quality.
