Description:
Objective:The objectives of the Center are to: (1) test the efficacy of an intervention for the primary prevention of lead toxicity, as measured by lead concentration and neurobehavioral functioning at 36 months of age; (2) serve as a model to evaluate the adverse effects of exposures to multiple prevalent toxicants among fetuses and children; (3) test meconium levels as a biomarker of fetal exposure to numerous toxicants; and (4) provide exposure and risk assessment data for residential pesticides.
Exposure to numerous environmental agents, including lead, mercury, polychlorinated biphenyls, and environmental tobacco smoke (ETS), has been linked with adverse neurobehavioral effects. Still, the ideal biomarker for measuring in utero exposure to specific toxicants has not been established, and the adverse effects of many potential neurotoxicants have not been tested rigorously. Fetal exposure typically is measured with self-reported surveys, maternal blood and urine, or cord blood. In contrast, meconium as a biomarker is a noninvasive method to test simultaneously for cumulative exposures to numerous toxicants. It is unclear, however, whether conventional biomarkers or meconium levels are more predictive of the adverse effects linked with specific toxicants. For lead exposure, emerging data indicate that our efforts should emphasize primary prevention, but the efficacy of lead hazard controls is uncertain, especially for children with low blood lead concentrations.
We propose to conduct a cohort study of 400 children, followed from less than 16 weeks’ gestation to 36 months of age, to examine the effect of low-level exposures to prevalent neurotoxicants. Endpoints include behavioral problems, such as conduct disorder and features consistent with Attention Deficit Hyperactivity Disorder, cognitive deficits, and hearing loss. We also will conduct a nested, randomized, controlled trial to test the efficacy of lead-hazard controls on the development of adverse neurobehavioral effects. We will test the following hypotheses:
(1) Children in the Lead Reduction Group will have lower blood lead levels, significantly higher cognitive scores, less hearing loss, and fewer behavioral problems than the Control Group at 36 months of age.
(2) Fetal and postnatal exposures to pesticides, ETS, and lead are associated with adverse neurobehavioral effects, growth delay, and hearing loss in early childhood.
(3) Fetal exposures are less predictive of the adverse effects of toxicants on cognition, behavioral problems, and hearing, compared with the same toxicants in meconium.
Progress Summary:The Cincinnati Children’s Environmental Health Center has had a productive year. The coordinating or administrative activities of the Center are described in this progress report. The Center hosted an international pooled analysis for lead-exposed cohorts to study intellectual impairments at blood lead levels below 10 μg/dL. The findings of our National Institute of Environmental Health Sciences (NIEHS)-funded Rochester Longitudinal Study found substantial lead-associated intellectual impairments at blood lead levels below 10 μg/dL (Canfield, et al., 2003). To test these surprising findings in a larger cohort, we convened this international collaboration, which is funded by the Centers for Disease Control and Prevention, the U.S. Environmental Protection Agency, and NIEHS. The collaboration involves more than 1,300 children enrolled in 7 prospective longitudinal cohort investigations from 4 continents. We are pooling the raw data to test whether there are adverse consequences of lead exposure on intellectual abilities at blood lead levels below 10 μg/dL. Further, we are testing whether the relationship is nonlinear and if there are greater impairments at blood lead levels below 10 μg/dL compared with higher blood lead levels. Our analyses are nearing completion, and we anticipate that we will have results in the next 12 months.
The Center has continued to play an important role in training researchers and clinicians in children’s environmental health. During the past year, we became one of the four sites for the Ambulatory Pediatric Association’s Pediatric Environmental Health (PEH) Fellowship Training Program. We currently have two pediatricians in our PEH training program. We also have continued to play a key role in the NIEHS-funded National Research Service Award training program in Molecular Epidemiology for Children’s Environmental Health (MECEH). Dr. Lanphear is the coordinator for the physician trainees in the MECEH training program.
In collaboration with Drs. David Rosner and Gerald Markowitz, our Center created a Web site on deceptive advertising by the lead industry. It includes an extensive exhibit of lead paint advertisements and the history of how the lead industry targeted children in its advertisements. “Living on Earth,” the National Public Radio weekly environmental science show, is doing an in-depth report of the Cincinnati Children’s Environmental Health Center. “Living on Earth” received a large grant from the National Science Foundation to do a study of nine emerging areas of research. Our Center was the first selected to highlight emerging neuroscience research. The purpose of the show is to relate to the audience what it is like to conduct a research study—the excitement, the hurdles, and the experiences of the research staff and participants. The journalists have visited numerous times during the past 6 months and conducted interviews with the investigators, subjects, and research staff. A 1-hour show featuring the Cincinnati Children’s Environmental Health Center was broadcast on April 17, 2003. “Living on Earth” will continue to follow the progress of the studies over the next year or more.
Our community partner, the Better Housing League, created a video with the assistance of the PPS Advertising Group. The video contains three 30-second spots about the dangers of lead exposure to children. One focused on children living in homes built before 1978, another focused on homes built before 1978 that had been renovated, and the third focused on the effects of lead poisoning on behavior. The video encourages viewers to telephone the Better Housing League for information. The video was shown as a public service announcement throughout the Cincinnati metropolitan area over several months.
