A Transition Planning Resource for Life After High School

For students with disabilities and their families

My Future My Plan is a curriculum designed to motivate and guide students with disabilities and their families as they begin early transition planning for life after high school. It promotes positive attitudes and self-advocacy, and assists students, parents, and professionals to make the transition planning process more effective. The curriculum package – which may be used in home, school, and community settings – includes a videotape and discussion guide, a workbook for students, and a guide for family members and teachers. All materials are available in English and Spanish.

My Future My Plan Videotape and Discussion Guide

Finding motivation to do something can be difficult. “It’ll take too much time. I don’t know what to do. There’s no one here to help me.” These are common reasons for inaction. This videotape tells the inspiring stories of three students and how they overcome barriers to achieve their goals. Their stories of determination motivate other students to explore their own options for the future. A discussion guide helps students and families to prepare for and begin the process of active involvement in transition planning.


My Future My Plan Workbook for Students and Guide for Family Members and Teachers

The workbook for students offers a step-by-step guide to transition planning. It addresses students’ legal rights, career exploration, self-advocacy, who to turn to for support, and much more. The workbook includes profiles of students with a range of disabilities who share how the skills taught in the workbook helped them achieve their transition planning goals. The guide for family members and teachers offers suggestions on how to work with students to use the workbook, and includes additional information for family members about letter writing and estate planning.
