Medication Reconciliation Results—Admission

The slide has two charts, one for inpatient admission and a second for outpatients.

Chart 1 graphs compliance with medication reconciliation—inpatient admission—from September 2006 to August 2007. Definition: Documented compliance with recommended Medication Reconciliation upon inpatient admission (physician, nurse, and/or pharmacist). The baseline is 0% through the entire period. The fiscal year 2007 goal is about 90% compliance. Actual compliance was slightly below 40% from September 2006 until around April 2007, when a mandatory training program took place. Compliance rose rapidly to around 90% in June 2007 as was approaching 100% by August 2007.

Chart 2 graphs compliance with medication reconciliation—outpatient admission/arrival—from May 2007 to August 2007. Definition: Documented compliance with recommended Medication Reconciliation upon outpatient arrival (includes 20 departments). The baseline is 0% through the entire period. The fiscal year 2007 goal is about 90% compliance. Actual compliance was around 90% in May 2007 and rose to nearly 100% in August 2007.

Notes:

Medication reconciliation impacts all patients at NMH who receive medications.  Compliance is now exceeding goal due to significant PowerChart enhancements, multidisciplinary computer classroom training, on-going feedback, the support of the Medication Reconciliation Leadership Team and participation of physicians, nurses and pharmacists.
