Any and all contracts created from January 1, 2002, to the present, that commission, authorize and/or retain the firm Ernst & Young to provide work and/or consulting services for the Department of Labor and all agencies under its administration (henceforth referred to as “contracts”). Mr. Harkinson is also asking that copies of the contracts be in paper, electronic or CD format at the soonest possible date.

Any and all logs of correspondence between members of Congress and the Department of Labor between January 1, 2007 to February 28, 2007 including all replies. Mr. Narayanswamy is requesting this to be a wide search throughout the Department and prefer to receive it in electronic format.

  • All claims forms filed by or on behalf of injured or deceased contractor employees in Iraq covered by the Defense Base Act and the War Hazards Act or their survivors seeking benefits under the Act. Such records should include, but not necessarily be limited to the name, nationality and place of residence of claimant; employer, place of employment; nature of work; date of injury or death; specific details of the cause of the injury or death (including illness, homicide, suicide and self-inflicted wounds); place of injury of death; and determination whether the claim qualifies under the War Hazards Act.
  • Electronic data file comprising each incident of death or injury including, but not necessarily limited to the name, nationality and place of residence of claimant; employer; place of employment; nature of work; date of injury or death; specific details of the cause of the injury or death (including illness, homicide, suicide and self-inflicted wounds); place of injury or death; and determination whether the claim qualifies under the War Hazards Act.
  • A record layout for the dataset is also requested.
  • Electronic data file comprising compensation payments records for each claimant covered by the Acts as stated above.
