Please have the case number ready when calling Customer Service. Lines are often busy so routine inquiries about case status and payment of compensation benefits should be routed to the Interactive Voice Response System.  For contact information regarding medical authorization or bill payment (including travel and reimbursements), click HERE.

Please ensure that the nine digit case file number is written on each page of any piece of correspondence sent to our office.

As noted above, all forms CA-1, CA-2, CA-2a, CA-16, and CA-7 should continue to be mailed to the office's physical address instead of the central mail facility in London, KY.

Our office is located on the eighth floor at 400 W. Bay Street in downtown Jacksonville, Florida.  We are staffed to accept visitors between 9 a.m. and 4 p.m. Monday through Friday. Appointments to review cases are necessary. Agencies wishing to review cases should have job offers for those cases. For more on agency case reviews, click HERE.

Imaged Cases are available for direct viewing in our Contact area by agencies with previously scheduled appointments; however, any injured worker wishing to see his or her case may request a copy of it by writing the first address listed above.
