COUNT I

Casey D. Stewart, on or about September 10, 2001, did, with intent to defraud Employee's Protection Association, make or utter a written instrument, that is: check # 1017, said instrument being of the following form:

in such a manner that said instrument purported to have been made by another person, namely: Debra Jean Stewart;

COUNT II

Casey D. Stewart, on or about September 10, 2001, did, with intent to defraud Employee Protection Association, make or utter a written instrument, that is: check # 1175, said instrument being of the following form:

in such a manner that said instrument purported to have been made by another person, namely: Debra Jean Stewart;

COUNT III

Casey D. Stewart, on or about September 10, 2001, did knowingly exert unauthorized control over the property, that is: checks, of Employee's Protection Association, with intent to deprive Employee's Protection Association of any part of the value or use of said property;

all of which is contrary to statute and against the peace and dignity of the State of Indiana.
