The measures are grouped under Quality of Care and include Process measures and Outcome measures. Difficult-to-change factors include patient risk of diabetes, under which is listed biological and socioeconomic. Feasible-to-change factors include patient ability to self-manage behavior, knowledge, and motivation, provider ability to manage diabetes, under which is listed incentives, knowledge, educational tools, and referral networks, and external environment, which includes disease prevalence, health care professionals, health care organizations, health care resources, government resources, government leadership, and QI programs.
