The purpose of this notice is to allow 60 days for public comment in the Federal Register preceding submission to OMB. This process is conducted in accordance with the Paperwork Reduction Act of 1995. The following summarizes the information collection proposal submitted to OMB: Type of Request: Reinstatement, without change, of a previously approved collection for which approval has expired. Originating Office: The Bureau of Consular Affairs. Title of Information Collection: Affidavit Regarding Change of Name. Frequency: On Occasion. Form Number: DSP-60. Respondents: U.S. Citizens and Nationals. Estimated Number of Respondents: 75,000. Average Hours Per Response: 15 minutes. Total Estimated Burden: 18,750 hours.
