So, How'd We Do?

At the DOT Disability Resource Center, our goal is to provide our employees and applicants with the accommodations and supports they need to be able to fully participate in the Department's work, programs and services. If you've recently received a service from us, someone from our office will be asking you to complete one of the following forms, so we'll know what worked well and where we might improve.

We have two feedback surveys; one for employees and applicants, and one for managers. If you are the person who received the service from us, please fill out the survey for employees and applicants. If you are the manager of someone who received a service, please fill out the manager's form.

Each form is available in two versions. One version is a fillable form in Microsoft Word format, and the other is a plain text document in Microsoft Word format. To use the forms, please click the appropriate link below and save the file locally to your computer. Open the document in Microsoft Word, and complete the form.
