correct their contact information included on the database may do so by calling
toll free 1.866.444.EBSA (3272) and submitting a
Verification of Contact Information
Form. Submission of this form will be used to provide updated contact
information to plan participants and will not be considered a formal change to
official plan documents filed with the Department.
EBSA is providing this contact information as a
public service, and does not guarantee its accuracy or completeness.
Employees/plan participants who rely on this contact information should ensure
that they are communicating with an official representative of the employer or
plan sponsor before providing personal information, such as a Social Security
number.
