1. What is the procedure to introduce a new form or request changes to an existing one? For a GSA form, you would fill out and obtain needed signatures for a GSA form 2192 to request forms management services. For a Standard (SF) or Optional (OF) Form to be used by all agencies, you would fill out and obtain needed signatures for a Standard Form SF152. Your request should then be submitted to Reza Motamedamin, Forms Policy and Management Team, Office of Governmentwide Policy (MEC), Room 2218, 1800 F Street NW, Washington, DC 20405. Former federal employees may request a copy of their final Standard Form 50, Notification of Personnel Action (SF 50), a complete copy of their official Personnel Folder (OPF), or certain other forms by writing to: National Personnel Records Center 111 Winnebago Street St. Louis, MO 63118-4126 The Privacy Act of 1974 (5 USC 552a) and this agency require a signed and dated written request for information from Federal records. Only written requests for records may be made. No calls, emails or faxes to this office regarding these requests will be accepted. There is an incompatibility between Adobe and some versions of Internet Explorer. Try right-clicking on the form link and use Save Target As... to save to the desktop. Usually opening the form outside of the browser will correct the problem.
