To assist customers in complying with mandatory reporting requirements, PHMSA provides a variety of forms in various formats.

In some cases, online reporting capability is available. Other formatted forms can be printed and filled out manually. Compressed formats contain a Microsoft Word template which may be stored on your hard drive. The PDF files may be saved directly to your hard drive by clicking your right mouse button over the link and selecting "save link as."

Microsoft and Adobe (PDF) provide free document readers that can be used to view or print files. Visit the PHMSA Accessibility pages (also linked at the bottom of every page) to download a reader or read more about Accessibility and Section 508.
