Yes, there are several ways to report.
Perhaps the easiest method is to report over the Internet. For this method you will need your User ID and Password, which are located below the address label on your report form. You may also call 1-800-772-7851 (press "(2)") to get this information. If you prefer, you may fax your report to 1-800-447-4813.
