8.  Besides mailing the form back, are there alternative methods of reporting?

Yes, you may fax your completed form to our National Processing Center at 1-800-447-4613.

9.  Are estimates acceptable?

Yes, carefully prepared estimates are acceptable if book figures are not readily available. In fact, we would prefer to receive timely estimates rather than late book figures.

10.  I reported estimates last month; do you want my book figures now?

Yes, we always want to improve our data. You can enter the previous month¿s book figures, along with additional comments, in the remarks section on the following month¿s report form.

11.  Why do you need to know if I own other companies or if another company owns my firm?

So we are not duplicating data. This might occur if you are affiliated with another company that may already be in one of our surveys.

12.  If I need another copy of the questionnaire(s), how can I obtain one?

You may call our National Processing Center on 1-800-772-7852 weekdays from 8:30 a.m. to 5:00 p.m. EST to request a replacement copy. You can also find this form on our website.

13.  What if I need more time?

Due to the timeliness of this report, it is important that forms be returned as soon as your data becomes available. However, should you require an extension, contact staff at our National Processing Center on 1-800-772-7852 weekdays from 8:30 a.m. to 5:00 p.m. EST.

14.  Can I be paid for completing the survey?

No. The law authorizing this survey (title 13, U.S. Code) does not authorize funds for payment.
