2) Year 2000 rates use US Census Bureau monthly postcensal estimates of the civilian noninstitutional population as July 1, 2000 and are available at the Census Bureau Internet site: http://www.census.gov/popest/archives/1990s/nat_monthly_noninstitutional/e0000smp.txt. Figures have been adjusted for net underenumeration using the 1990 National Population Adjustment Matrix. Regional estimates were provided by the Division of Health Interview Statistics (DHIS), National Center for Health Statistics, and are also based on U.S. census estimates of the civilian noninstitutionalized population as of July 1, 2000.

3) Year 2001 rates use U.S. Bureau of the Census estimates of the civilian noninstitutionalized population as of July 1, 2001 and are based on data from Census 2000. Year 2001 estimates of MSA/non-MSAs are calculated from data provided by the Office of Research and methodology (ORM) and Division of Health Interview Statistics (DHIS), National Center for Health Statistics, and are based on estimates of the civilian noninstitutionalized population as of July 1, 2002 using Census 2000 data, with adjustments to 2001 totals.
