Initial Summary:   Firefighter Bajusz was responding to a recreational vehicle fire (connected to a mobile home (charging), electrical contributing to ignition) on Conch Key in his personally owned vehicle when on Grassy Key, after pulling over to the right portion of his lane of travel, Bajusz attempted a U-turn and was struck by another vehicle coming up from behind (large utility truck). Bajusz suffered serious injuries with ongoing complications and remained in the hospital for just over six months. Firefighter Bajusz asked to be taken to his home where, the following day, he passed away from his injuries. Initial Summary:   Firefighter Barker was operating fire apparatus and returning from a propane gas-grill fire, with exposures, that had been successfully mitigated. As the apparatus came down a hill on West Hill Road A, at approximately 3,550 linear and 270 vertical feet from the crest, it came into a left-hand zigzag curve where the apparatus left the right side of the roadway coming to its final resting position having struck one or more trees. Barker was reported to have been ejected from the vehicle. The cab area of the apparatus was heavily damaged; the water tank separated from the vehicle and was several feet further down the road in the opposite lane of travel. Firefighter Barker, the only department member onboard the apparatus, was transported by Erway Ambulance to Arnot Ogden Medical Center in Elmira but did not survive his injuries. The cause of the accident is still under investigation by the Chemung County Sheriff's Office. Initial Summary:   Assistant Chief Burchett, 33, was fatally injured while working at the scene of a minor motor vehicle accident on U.S. 460 in Johnson County near the Magoffin County line. While on the scene, Chief Burchett was evaluating and assisting a female patient, who also happened to be pregnant, when a coal truck that was coming over a hill towards the accident scene braked in icy conditions and struck both Burchett and her patient, Erica Brown, after first striking a police cruiser. Brown was pronounced at the scene. Police say she had not appeared to have suffered any major injuries in the initial vehicle accident. Burchett was transported to Paul B. Initial Summary:   Shortly after he and other members of the fire department had brought a fire in a vacant residential structure under control, Firefighter Crockett, for a cause still to be determined, collapsed while on a hose line working overhaul. Initial Summary:   Incident Description: An air tanker (Neptune Tanker 09) under contract to the United States Forest Service crashed shortly after take-off from the Reno-Stead Airport. The crew of three was lost in the incident. The aircraft, a Lockheed P2V-7 Neptune, fitted with two piston and two jet engines, was operated by Neptune Aviation Services of Missoula, MT. It had been released from fighting the Burnside Fire in Hope Valley, CA (USNG: 11S KC 454 961), and was departing Reno-Stead on a new assignment to fight another fire in Calaveras County, CA (USNG: 10S GH 08 34). Preliminary statements from NTSB and witnesses indicate that a jet engine fire engulfed one of the tankers wings in flames moments after take off, sending the plane rolling into the ground. Investigators recovered several large pieces of metal beginning about one-quarter mile north of the runway that appear to have come from the burning engine. Initial Summary:   Firefighter Hales had returned home from working a twelve hour shift on three outdoor fires caused by lightning when within a couple hours he left his residence again with his 14 year old daughter in their privately owned vehicle. A short distance from the home, the vehicle left the roadway and crashed after (according to the autopsy report) Hales suffered a cardiac arrest. The young lady ran back home a half mile to call 911, however, Firefighter Hales did not recover and passed away.

Initial Summary:   Firefighter Holmes and another firefighter (Lt. Scott King) were attempting to rescue a 44-year-old female resident from the second floor of a burning duplex, when conditions in the building rapidly deteriorated and the firefighters became trapped. The two firefighters were rescued by a rapid intervention team and transported to the hospital. Firefighter King has since been released from the hospital, but Firefighter Holmes, after several days in critical condition at the Mercy Hospital Burn Unit, succumbed to his injuries in the morning hours of 03/05/2008. The female resident of the structure did not survive the fire. According to FirefighterCloseCalls.com, the fire has been ruled accidental by the State Fire Marshal, sparked by a hair dryer on the living room couch on the first floor.

Initial Summary:   Incident Description: Lt. Jones was exercising behind Station 10, 2711 Sanderford Road (USNG: 17SQV15825789). When the alarm rang for a respiratory distress call, he ran back inside to respond. At the time, Jones complained to his Captain that he thought he had pulled his left hamstring. He did not complain about the pain anymore during the course of the day which included another medical assist call (shooting). That night, Lt. Jones not feeling well, called his Captain on the phone from his cubicle @ 2355hrs. The Captain found Jones lying on a sofa in extreme pain. He was sweating, complaining of indigestion and pain in his left leg. Station 10 personnel took Lt. Jones' blood pressure which was 225/110 in his right arm. Upon examination of his left leg, it appeared flaccid and personnel could not find a pulse in Lt. Jones' left foot. Garner EMS responded to assist Lt. Jones at 2357hrs, at which time he was transported to Wake Medical Center. Tests indicated Lt. Jones was suffering from a torn aorta. Lt. Jones was later transferred to UNC Hospital in Chapel Hill where he passed away from his injury on 01/22/2008.

Initial Summary:   At approximately 1900hrs, Firefighter Leduc responded to a report of a man who went overboard from a boat into Stafford pond. Leduc was part of a dive team who was searching for the lost subject when he suddenly had difficulty and went into cardiac arrest. Initial Summary:   Los Angeles Firefighters were summoned to investigate smoke in the 8800 block of South Sepulveda Boulevard. At approximately 1440hrs, an explosion occurred at a nearby building, causing injury to two male Firefighters and one civilian. Firefighter Brent A. Lovrien, assigned to the 'A' Platoon at Fire Station 95 since October 2005, died shortly after arrival at the Centinela Freeman Regional Medical Center, Marina Campus. Engineer Anthony J. Guzman, age 48, an 18 year veteran of the LAFD assigned to the 'A' Platoon at Fire Station 95 since February 2002, suffered multiple fractures and facial trauma. He was transported to the UCLA Medical Center in Westwood, where he remains in serious but stable condition following surgery. Preliminary reports indicate the civilian was treated at the scene and released. As dictated by Fire Department policy, a formal and detailed multi-agency investigation is underway to determine the exact nature of the explosion, as well as the precise cause and manner of Firefighter Lovrien's death. Initial Summary:   On Friday, June 27, 2008, Firefighter MacDonald was fighting a fire on the north rim of the Grand Canyon National Park when he was bitten by an insect and taken to a nearby hospital for treatment. On Sunday, June 29, 2008, he suffered anaphylactic shock from the antibiotics and was being flown to the Flagstaff Medical Center in Arizona. Initial Summary:   Firefighter Maddy and his department had responded to a series of arson fires, the community was placed on curfew and a fire watch was established. Approximately 16 hours after working the fire responses and finishing his assignment on the fire watch, Maddy was rushed to Ball Memorial Hospital, suffering from chest pains. He passed away a few hours later from an apparent heart attack. Two suspects, a juvenile and an 18-year-old male, have been arrested in connection with the reported 21 fires that included four cars, a recreational vehicle inside and a boat sitting outside of a metal garage, numerous trashcans, a moped and a church bus.

Initial Summary:   Gert Marais of Fort Benton, Montana, was killed when his Single Engine Air Tanker (SEAT) went down just after dropping a load of slurry onto portions of a wildfire on Fort Carson Downrange. The SEAT, operated by Areo Seat of Sterling, CO, was working under contract for the Colorado State Forest Service on a wildfire that had grown rapidly during the day, threatening buildings within and along the western flank of the Downrange training area. A mandatory evacuation was ordered for areas west of State Hwy 115 from Barrett Road (13S EC 0741 7123) north to Ft. Carson Gate 5 (13S EC 1576 8570). There were no controlled burns taking place on the Downrange; the cause of the fire is under investigation. The National Transportation Safety Board and Federal Aviation Administration are investigating the SEAT accident. Initial Summary:   Firefighter McAtee responded to the fire station and drove apparatus to the scene of a chimney fire. Upon arrival, he reported to the Fire Chief, his son, and told him that he thought that he pulled a muscle opening the apparatus bay door when leaving the station. Later McAtee was observed holding his chest and when asked, indicated that he thought that perhaps it was the extremely cold weather that was causing him discomfort. Firefighter McAtee declined to be sent for evaluation or treatment and returned the apparatus to the station. He returned home and was witnessed by his wife again holding his chest, but stated it was either a pulled muscle or from the extreme cold. At 1629hrs, Fire Chief McAtee was summoned to Firefighter McAtee's residence, finding him unresponsive. CPR was initiated on scene by the Fire Chief and care was continued through transport to the local hospital where Firefighter McAtee succumbed to his injuries.

Initial Summary:   After they had discovered a fire in the basement of their home, a family safely evacuated a two-story, stucco wood-frame single family residence in Forrest, IL, and subsequently called 911. Firefighters from seven communities in the Forrest area, including Fairbury responded. Initial Summary:   Incident Description: Lieutenant Picozzi became trapped when fire conditions rapidly deteriorated in the basement of an unoccupied single family residential structure. Three other firefighters were injured, one taken to the Chester-Crozer Hospital Burn Unit and reported to be in fair condition; two others treated at the hospital for smoke inhalation and released. The cause of the fire is under investigation but is thought be related to two large trees near the residence falling on a 13,000-volt primary distribution line. Initial Summary:   Second Assistant Chief Thomas passed away from injuries sustained when he was struck by a semi tractor-trailer on a stretch of US Highway 17 adjacent to Marine Corps Base Camp Lejeune. Camp Lejeune contacted the Sheriff's office shortly before 0500hrs requesting assistance with traffic control because of extreme low visibility conditions on the roadway due to smoke from a controlled burn on the Base combining with fog. Thomas, along with Onslow County Deputy Sheriff Steven Boehm, 36, who was also struck and killed by the tractor-trailer, had been directing traffic when a series of 8 vehicle collisions took place in the northbound travel lanes of the highway. Chief Thomas, Boehm and others heard and responded to calls for help, searching through the fog and smoke when unfortunately they were struck by the tractor-trailer. The tractor-trailer then struck several other vehicles sending another deputy to the hospital; he was later released. Initial Summary:   On Saturday, February 16th, while operating fire apparatus enroute to a structure fire (cabin), Captain Tomaselli suffered a stroke. Tomaselli sideswiped several trees but was able to keep control of the engine until he reached a clearing, bringing the apparatus to a stop. He was airlifted to Loma Linda University Medical Center, where he underwent emergency surgery. Tomaselli remained in the hospital but was unable to recover from his injury, passing away late in the evening of February 21st.

Initial Summary:   At approximately 1845hrs, shortly after he finished a live fire training exercise at the Oklahoma State University Fire Service Training Center, Firefighter Topping was found collapsed and unresponsive by other firefighters who immediately began to administer CPR. Topping was transported to the Stillwater Medical Center; all efforts to revive Firefighter Topping were unsuccessful. The cause and nature of the fatal injury are still to be reported.

Initial Summary:   Incident Description: An air tanker (Neptune Tanker 09) under contract to the United States Forest Service crashed shortly after take-off from the Reno-Stead Airport. The crew of three was lost in the incident. The aircraft, a Lockheed P2V-7 Neptune, fitted with two piston and two jet engines, was operated by Neptune Aviation Services of Missoula, MT. It had been released from fighting the Burnside Fire in Hope Valley, CA (USNG: 11S KC 454 961), and was departing Reno-Stead on a new assignment to fight another fire in Calaveras County, CA (USNG: 10S GH 08 34). Preliminary statements from NTSB and witnesses indicate that a jet engine fire engulfed one of the tankers wings in flames moments after take off, sending the plane rolling into the ground. Investigators recovered several large pieces of metal beginning about one-quarter mile north of the runway that appear to have come from the burning engine. Initial Summary:   Captain Whiten had been assisting other instructors at a structural burn session and had led several training burns when he decided to go to rehab. While at rehab, his vitals were determined to be good; he sat in front of a cooling fan, and then proceeded to an area to relax. Following that, he was found down, suffering from a medical emergency. He was transported to Mon Valley Hospital where he passed away from a cause still to be reported.

Initial Summary:   After returning from a mutual aid call, Fire Chief Wichlacz was in the station when he was accidentally pinned between a bay door and a fire tanker when a privately owned pickup truck being moved by another firefighter reportedly accelerated out of control and crashed through the door. Chief Wichlacz passed away from injuries sustained in the incident.
