IDES Revision Instructions

If your original submission was completed using the Internet Data Entry System, follow the instructions below to complete a revision:
  • If you are not making any changes to the form and would like to print a copy, then you may log into your online Form using your Report ID and Password
  • If you need to make changes to a form, which has already been finalized, you may use the 'unlock' feature from the main menu. Please review the Internet Data Entry System Instruction Manual for information on 'unlocks'.

  • Please send all pages of the revised Form SF-SAC and, if needed, new audit components, to the address listed on the top of Page 1 of the form.
If you did not use the Internet Data Entry System to complete your original Form SF-SAC, then follow the instructions below to complete a revision:
  • Print out a blank copy of the Form SF-SAC from our website at http://harvester.census.gov/fac/collect/formoptions.html (Option #2)

  • Write "REVISION" at the top of Page 1 (Form SF-SAC).

  • Complete Page 1, sign and date (both auditee and auditor).

  • For all other pages, enter the desired changes (you are not required to complete all questions on Pages 2,3, and 4).

  • If changes include Page 3 Federal program data, be sure to type the entire program line.

  • If changes to the Federal Program data affect the Total Federal Awards Expended (TFAE), include the new total in the TFAE box.

  • Please send all pages of the revised Form SF-SAC and, if needed, new audit components, to the address listed on the top of Page 1 of the form.

  • Please include a cover letter briefly explaining the changes you have made to your submission.
