Improving Reactive Hazard Management

Summary
Following its investigation of a serious reactive incident at Morton International, the Board initiated a comprehensive review of all reactive hazards in the U.S. The investigation found that current standards, regulations, and good practice guidelines do not adequately cover reactive hazards. The investigation also found that improving reactive hazard management requires both regulators and industry to address the hazards from combinations of chemicals and process-specific conditions rather than focusing exclusively on the inherent properties of individual chemicals. Furthermore, the investigation found that existing sources of incident data are not adequate to identify and analyze the number, severity, and causes of reactive incidents. The CSB issued safety recommendations based on these conclusions.
