Incident Description
In the aftermath of the March 1998 nitrogen asphyxiation incident at Union Carbide in Hahnville, LA, the Board initiated a review of similar incidents nationwide. The CSB study identified a total of 85 incidents that occurred in the U.S. between 1992 and 2002 and involved exposure to a nitrogen-enriched atmosphere. Together these incidents caused 80 deaths and 50 injuries. The CSB developed a safety bulletin on nitrogen ashyxiation hazards, highlighting a variety of good practices to avoid such incidents.

Investigation Status
CSB's safety bulletin was issued in June 2003.
