d Children with both public and private insurance coverage are placed in the private insurance category.
e As defined here, public health insurance for children consists mostly of Medicaid or other public assistance programs, including State plans. Beginning in 1999, the public health insurance category also includes the State Children’s Health Insurance Program (SCHIP). It does not include children with only Medicare, Tricare or CHAMP-VA.
