Appendix 1: Conducting a Health Literacy Assessment—Flow Chart (Text Description)

This is a flow chart in a matrix format with three columns representing the three parts of the pharmacy health literacy assessment.

In the left column, under "Part I. Assessment Tour of the Pharmacy," there are three steps listed vertically. The first is Gathering Information: "Identify and train at least two objective auditors," and "Conduct at least two assessment tours at each location." An arrow points down to Analyzing Results: "Analyze results using spreadsheet or statistical software program." An arrow points down to Interpreting Results: "Interpret results (identify areas of strength and weakness as indicated by objective auditors)."

In the middle column, under "Part II. Pharmacy Staff Survey," the same three steps are listed. The first is Gathering Information: "Identify sample and distribute survey." An arrow points down to Analyzing Results: "Analyze results using spreadsheet or statistical software program." An arrow points down to Interpreting Results: "Interpret results (identify areas of strength and weakness as indicated by staff)."

In the right column, under "Part III. Patient Focus Group," three steps are repeated. The first is Gathering Information: "Recruit focus group participants from the pharmacy," and "Conduct at least two focus groups with 9-12 participants each." An arrow points down to Analyzing Results: "Transcribe recorded discussion," "Summarize notes," "Identify common themes," and "Analyze demographic data." An arrow points down to Interpreting Results: "Interpret results (identify areas of strength and weakness as indicated by patients)."

One final step, Using Results, spans all three columns along the bottom of the chart, with arrows pointing down to it from each column. This step includes "Develop story of pharmacy's status in serving limited literacy patients," and "Tell the story to the organization (include suggestions for improvement)."
