Obesity visit template-Risk factor assessment

Screen shot of the risk factor assessment page. It provides a short questionnaire to ask the child and parent regarding diet. The questions ask about use of: juice/soda, fast food/prepared food, vegetables/fruits, diet variety, meal/snack timing, portion size, TV use, active play, and if the biological mother and/or father are overweight. It also includes several suggestions such as bring lunch to school, reduce number of daily sodas, and reduce hours of TV-watching.
