Key Findings of the Demonstration by Urban Institute

  1. Providers liked the concept, clients were positive about the card.
  2. Privacy/Security was not a problem for clients.
  3. Clients felt empowered in using the card and managing their child's health data.

Phase I Card

This slide displays an example of the front and back of a Health Passport Card. Like a credit card, the individual's name appears on the card, along with an identification number.

Legacy Integration

The HPP Card captures data from each legacy system and has the ability to send new information to the legacy system

This slide features a diagram of how the legacy systems work together. The HPP card is in the center, surrounded by the different systems, with arrows illustrating that data can go both ways from a legacy system to the card. The systems surrounding the card are (Clockwise from top right):
