Project Description: The objective of this grant is to develop collaborative management of type 2 diabetes in black Americans. This objective is to be accomplished by understanding patient, provider, and systems level barriers to the effective management of diabetes in primary care settings and the development of a collaborative model of care that has the potential to eliminate these barriers. This research has three specific goals:

  1. To identify the knowledge, beliefs, and attitudes of black patients that impact their diabetes outcomes.
  2. To identify impediments to improved diabetes outcomes in blacks that are attributable to health care providers.
  3. To identify external factors that impact on diabetes outcomes in blacks.

Career Goals:  Dr. Egede plans to develop effective interventions to improve patient-provider communications and reduce racial/ethnic differences in health outcomes in patients with type 2 diabetes. This will be accomplished by submitting new investigator initiated grants to Federal and non-Federal funding agencies based on the findings from the preliminary data collected as part of the career development award. These R01 and R21 grants will be used to develop and test innovative interventions to increase trust between minority patients with type 2 diabetes and the health care system, interventions to improve health provider-patient communication, interventions to decrease psychosocial barriers to effective diabetes self-management, and health systems interventions to improve quality of diabetes care for ethnic minorities.

Progress to Date:  The patient and provider survey components of the project have been completed. Multiple publications based on the surveys have been published. Clinic and community based interventions to improve quality of care for black Americans with diabetes are being developed. A pilot intervention to increase physical activity levels and improve self-care behaviors in black American women is in the process of testing in community sites. The awardee has competed and successfully obtained grant funding from the Department of Veterans' Affairs (VA) Health Services Research and Development Service (HSR&D), and has two R01 grants in the review process. The awardee was recently awarded an institutional grant to establish a Center for Health Disparities Research, which will further advance the objectives of the K08 award.

Future Plans: Data analysis will continue. In the next year, the awardee will continue to write R21 grants to pilot test the interventions that are being developed. It is anticipated that R01 grants based on the pilot interventions will be submitted within the next two years. Additional focus groups will be formed to examine the beliefs and attitudes about diabetes self-management, barriers to receiving culturally sensitive care, perceptions about health care providers and the health care system, and beliefs about patient autonomy. Multiple manuscripts are in press. The grantee will continue his limited clinical activities and teaching as well as develop Federal grant applications.
