Fields allow entry of data for Type of Facility (Alternate Care Site or Hospital/Medical Institution); Site or Institution Name; Name of Chief Administration or Contact; Street Address, City, State, Zip Code, Telephone Number, County; American Hospital Association (AHA) Identification Number; If Alternate Care Site, Name of Operating Facility; Longitude and Latitude; Metropolitan Statistical Area (MSA) Code, and whether or not the facility is an NDMS or Tricare Participant. Sample Data is entered for a fictitious site, Lowry AFB Hangar 2.
