a Usual source of care is a composite of two questions asked: Is there a place that (sample child) usually goes when (he/she) is sick or you need advice about (his/her) health? Is that a doctor's office, emergency room, hospital outpatient department, clinic, or some other place? Respondents who answered "no" to the first question were coded as no usual source. Respondents who answered "yes" to the first question but listed the emergency room as the usual source or answered that the child does not go to one place most often were also coded no usual source.
bPoor refers to incomes below the Federal poverty line; near poor, over the poverty line to just below 200 percent of the poverty line; middle, 200 percent to just below 400 percent of the poverty line; and high, 400 percent of the poverty line and over.
