3. Implementing the Chronic Care Model into Practice

The Chronic Care Model is designed to move medical practice from an acute care delivery system to one more appropriate for the patient with a chronic illness.

What follows are examples of how the sites in the Academic Chronic Care Collaborative implemented planned visit models and new clinical information systems that served this purpose.
