The Department of Health and Human Services (HHS), Office on Women's Health (OWH), requires statewide evidence-based educational interventions (using The Heart Truth Professional Education Campaign materials) to expand the role of primary care in the prevention and treatment of heart disease risk factors, and the diagnosis of the metabolic syndrome and heart disease among women aged 40 to 75, with specific emphasis on blood pressure control, tobacco cessation, lipids control (including triglycerides, LDL-C, HDL, total cholesterol), diabetes control (including testing, monitoring, nutrition counseling, and foot care), weight loss or control, and symptoms of heart disease. Preference will be given to interventions in fifteen states with the highest heart disease mortality rates in women.



The Contractor will conduct standard educational interventions using The Heart Truth Professional Educational materials, evidence-based clinical practice assessments and brief interventions, and standard risk factor treatments for women to expand the role of primary care in the prevention of heart disease in women (ages 40-75 years). The Contractor shall use culturally-relevant approaches in designing their educational and practice based interventions. The Contractor may select one or more states (Alabama, Arkansas,Delaware, District of Columbia, Georgia, Kentucky, Louisiana, Mississippi, Missouri, New York, Ohio, Oklahoma, Tennessee, West Virginia, and Michigan) for these primary care interventions. In addition, one or more primary care provider specialties may be the focus of the interventions.



PLEASE BE ADVISED THIS IS NOT A GRANT. This requirement will be solicited and awarded under FAR Part 15, Contracting by Negotiation, procedures. The award will be a contract. The Government anticipates making three awards but reserves the right to make up to four awards. The period of performance will be 36 months starting from the date of award consisting of a base year with two, one year options.



The solicitation will be made available electronically on or about 15 days from the date of this publication. The internet address for downloading the solicitation is http://www.fbo.gov. Any notification of changes to the solicitation (amendments) will be posted on the internet. Interested parties are responsible for monitoring the site to assure they have the most current information regarding this aquisition. The anticipated release date for the solicitation is July 11, 2008. The anticipated contract type is firm-fixed-priced.
